YOUTH GROUP PERMISSION & REGISTRATION FORMS

To ensure the safety of our youth during activities, permission is required from each youth's parent or guardian. Each form is provided in

  • Microsoft Word format which can be read and printed on a PC, Mac or smartphone usng Microsoft Office or compatible program
  • Adobe PDF format which requires Adobe Reader to read and print the form
 
Just print the form and have your parent or guardian sign it.  Then mail or fax the form to church. See contact us.
